Am I eligible?

Jackson Care Connect is a coordinated care organization (CCO) that connects you to physical, dental and mental health care and other services within the Oregon Health Plan (OHP).

Within the OHP, there are two plans that Oregonians can qualify for: OHP Plus and OHP Bridge. The differences in these plans are small and eligibility for both are based mainly on income levels. But there also could be other factors that determine eligibility for either plan.

Do I qualify?

The chart below shows the highest income levels you can have to see if you qualify for the Oregon Health Plan. But remember, this isn’t the only thing that decides if you’re eligible. There are other factors that aren't listed here. The best way to find out if you qualify is to apply. You might be able to get coverage, even if you were turned down before. There are several different ways to apply for OHP:

In-person assistance:

An “Assister” from OHP, or a helper at a community health center will guide you through the enrollment process.

What information and documents do I need when I apply?

Even when your income qualifies you for the Oregon Health Plan, the state asks you to provide certain documents.

Have these with you when you apply online, or bring them with you to an in-person event.

Which OHP application method is best for me?

The OHP application method you use depends on how much time you have, how much privacy you have at your location and when you need your coverage to start. Overall, working with an assister seems to take the least amount of time from application through enrollment and offers one-on-one help for any questions you may have.
